What microdosing means
In practice, microdosing typically refers to 0.125–0.25 mg of semaglutide weekly, or 1.0–2.5 mg of tirzepatide weekly. These doses are roughly one-tenth to one-quarter of the standard weight-loss range.
The metabolic case
Even at sub-therapeutic doses, GLP-1 agonists exert measurable effects on appetite, gastric emptying, and insulin sensitivity. For weight maintenance after a successful loss phase, this can be sufficient to prevent regain without the cost or side effects of full doses.
The longevity case
A separate argument extends GLP-1 use to metabolically healthy patients seeking improved insulin sensitivity, reduced inflammation, and possibly cardiovascular benefit. The mechanistic case is reasonable; randomized data in non-obese populations is limited.
Where the evidence ends
We have strong randomized data for therapeutic-dose GLP-1s in obesity and type 2 diabetes. We have observational data and physiology to support maintenance dosing. We do not yet have large randomized trials evaluating sub-therapeutic dosing as primary therapy in metabolically healthy adults.
Practical guidance
Microdosing is reasonable as a maintenance strategy after standard weight-loss therapy and as a tool for patients with insulin resistance or pre-diabetes who do not yet meet BMI criteria for standard dosing. It should always be physician-supervised, with periodic labs and a clear clinical rationale documented.
