Before any patient starts testosterone replacement, the question worth asking is whether lifestyle and supplement-based optimization can close the gap. Often it can; sometimes it can't.
What lifestyle interventions actually do
Sleep extension from 5 to 8 hours per night increases morning testosterone by roughly 15%. Losing significant excess body fat increases testosterone by 5–15%. Resistance training 3+ days per week produces modest but durable increases. These are not small effects in a healthy man with mild suppression.
What supplements actually do
Ashwagandha (600 mg/day of a standardized root extract) shows the most consistent evidence — typically a 10–15% increase over 8–12 weeks in stressed or training men.
