Thymosin alpha-1 (thymalfasin) is one of the most-studied peptides in clinical medicine globally. Approved for various immune indications in more than 30 countries — including in chronic hepatitis B, hepatitis C, and as an adjunct to cancer therapy — its U.S. use is primarily off-label, but the evidence base is substantial.

What it is

Thymosin alpha-1 is a 28-amino-acid peptide originally isolated from the thymus gland. The thymus is where T-cells mature; thymic involution begins in adolescence and progresses throughout life, contributing to age-related immune decline.

Mechanism

TA-1 acts primarily on the innate and adaptive immune systems via Toll-like receptor 9. It enhances T-cell maturation, increases natural killer cell activity, and modulates dendritic cell function. The net effect is improved immune competence without the broad immunosuppression seen with corticosteroids.

Where the evidence is strongest

Chronic hepatitis B and C — multiple randomized trials show TA-1 improves response rates when combined with antiviral therapy. Sepsis — randomized trials in critically ill patients show improved 28-day survival in some studies. Adjunctive cancer therapy — used internationally to support immune function during chemotherapy.

Where Valora uses it

Selectively, in patients with documented chronic viral reactivation (EBV, CMV), recurrent infections suggesting immune dysregulation, or post-acute COVID syndromes with measurable immune dysfunction. We do not use it as a generic "immune booster" without a clinical reason.

Typical protocol

1.6 mg subcutaneously, twice weekly, for an initial 3-month course. Response is evaluated clinically and with appropriate immune markers. Extended courses are used in selected patients with ongoing immune dysfunction.

Safety

TA-1 has an excellent safety profile across decades of international use. Side effects are minimal — usually injection-site reactions only. It is not appropriate for patients with active organ transplantation or certain autoimmune conditions.
